"My name is John H and I am a client of yours. Last Spring I spent the bulk of two months in two local inpatient facilities accumulating the expected large amount of charges and fees. Fortunately, my insurance plan covered the bulk of those expenses and my out-of-pocket payments were relatively small. However, I was informed by my insurer that three medical provider fees were rejected, either for erroneous or ill-timed submission.
